  Evaluation

The evaluation component tracks indicators, short, and long-term outcomes at each clinic site, school, community pharmacy, and collaborating agency. The evaluation plan consists of both process and outcome evaluation. The evaluation team maintains extensive databases and performs statistical analysis on the data gathered from each of the other project components. The team also functions as "internal" evaluators by administering the Partnership Self-Assessment Tool (PSAT) to identify its strengths and weaknesses, and by maintaining an activity log to track significant community changes that occur as a result of consortium effort.
